Responsibilities
- Maintaining records of all Sullivan County births
- Issuing birth records for all people born in Sullivan County
- Submitting data on Sullivan County births to the Indiana State Department of Health
- Maintaining records of all Sullivan County deaths
- Issuing death records for all persons who have died in Sullivan County
- Submitting data on Sullivan County deaths to the Indiana State Department of Health
- Providing records that are available to the public for genealogy searches
- Birth and Death Records date back to the years 1882 and 1898 respectively
Birth & Death Certificates
Birth and Death certificates can be purchased during normal business hours or, also may be requested by mail. When requesting by mail please print and fill out the proper application and return to our office with a copy of your ID and payment (no personal checks or credit/debit cards accepted).
- Certified Birth Certificate: $10 each
- Certified Death Certificate: $12 each
- Paternity Affidavit Processing: $20
